Proposal to the 11th Revision of the WHO
International Classification of Diseases (ICD-11) #2130
On 6 January 2019, I submitted the Complex Hierarchical Changes Proposal #2130 to the Committee for the 11th Revision of the WHO International Classification of Diseases (known as ICD-11). I signed the proposal with colleagues Prof. Jens Ove Andreasen (Copenhagen University Hospital, Copenhagen Denmark), Prof. Ulf Glendor (Malmö University, Malmö Sweden), and Prof. Lars Andersson (Malmö University, Malmö Sweden), three past Editors of the Wiley journal Dental Traumatology. We asked to reclassify Traumatic Dental Injuries generating a specific Block, NA10, possibly called Traumatic Dental Injury or Injury to Teeth .

After a Review Process of 34 months

this proposal has been approved as it is by the Classification and Statistics Advisory Committee (CSAC) of the World Health Organization on 29 October 2021.

This Complex Hierarchical Change will produce an important change in the ICD system. Indeed, Traumatic Dental Injuries, the fifth most frequent disease in the world, that was neglected by the past ICD classification systems and the Global Burden of Disease study, will have a specific block with specific codes in the ICD-11. Jens Ove Andreasen and Lars Andersson have in vain asked to fix this problem several times in the past. Sadly, both Prof. Jens Ove Andreasen and Prof. Ulf Glendor passed away during this process.

Scientific Publications and Research

Past Research Highlights
I am an internationally acknowledged expert in Epidemiology of Oral and Infectious Diseases. These are my main contributions to science. Contributions that changed the pre-existing scientific opinion and evidence are highlighted in bold character.

Study of risk factors for oral cancer
With Crispian Scully from the University College London, we found and reported that alcohol drinking is not an independent risk factor for oral cancer, but interacted with smoking and, possibly, betel quid chewing. Routine drinking subjects who are not exposed to other lifestyle risk factors are not at oral cancer risk. This was an important discovery, since alcohol drinking was considered and is currently considered a key and independent oral cancer risk factor.
Study on oral cancer screening
I found and demonstrated the unpredictability of visual screening in oral cancer early detection, which is considered the basis of several oral cancer control programs throughout the world.
Study on the association between Hepatitis C virus infection and oral lichen planus
With Crispian Scully we resolved the problem as to whether HCV infection is a risk factor for oral lichen planus or not. We found that HCV is a mild OLP risk factor, therefore, only patients chronically infected by specific HCV strains are at risk for OLP.
Antimicrobial activity against antibiotic-resistant microorganisms
With Stephanie Dancer, from the Glasgow Hospital, we found that enhanced cleaning of environmental surfaces with detergents has the same antibacterial effect than the use of disposable barriers or disinfectants on smooth surfaces against antibiotic-resistant microorganisms. This is an important contribution in infection control in hospital settings.
Transmission of antibiotic-resistant microorganisms in dental healthcare settings
With Johannes De Soet from the ACTA University of Amsterdam we found that dental healthcare workers are not at higher risk of acquiring and transmitting antibiotic resistant bacteria as previously conjectured, thus solving the problem of their control in dental healthcare settings.
Study of the Ebola outbreak in West Africa
With Crispian Scully we highlighted the crucial role of unapparent carriers in Ebolavirus Disease (EVD) transmission. This hypothesis was considered controversial but explained the spread of this infection to Healthcare Workers from Western Countries and predicted the ineffectiveness of the global EVD control strategy based on the control of people from West Africa in the airports.
Study of Legionnaires Disease in dental healthcare settings
I found that despite the frequent reports of dental unit water contamination with legionellae, the risk for the dental staff and patients to become infected and consequently develop Legionnaires Disease is negligible and hypothesized human-to-human transmission of Legionella pneumophila. Although many experts do not recognize this result, several international epidemiologists expert in Legionella infection did.
Oral fluid retraction into dental unit waterlines during dental therapy
Although this issue was already hypothesized in the late 80 s, we found an easy method to detect and cultivate oral streptococci in environmental water samples from dental unit waterlines, thus demonstrating that oral fluid retraction during dental therapy was more than a hypothesis.
Study of risk factors for dental trauma
With the contribution of Dr Giulia Cairella, nutritionist, we discovered in the 90 s that obesity is a risk factor for dental trauma. Since then, several studies were performed which confirmed this result.
Study on traumatic dental injury prevalence
During the project of revision of the Textbook and Color Atlas of Traumatic Injuries to the Teeth for the Fifth Edition edited by Jens O. Andreasen, Frances M. Andreasen and Lars Andersson, we assessed the World Prevalence of Traumatic Dental Injuries. With Lars Andersson, Ulf Glendor and Jens Ove Andreasen, we are working to include this condition in the list of the diseases considered by the Global Burden of Disease and Injury Study (GBD), and the International Classification of Diseases (ICD).
